CONSENT FOR SOUND HEALING:

 

Sound Healing Class Description: This class is for adults 18 years and older, and will be limited to
12 adult participants per class. In this class, the instructor will lead participants in sound healing
(utilizing 7 chakra-tuned Crystal singing bowls) paired with guided breathwork. Sound healing
has been shown to improve well-being and sleep and alleviate anxiety, stress, and physical pain.

**I UNDERSTAND THAT DUE TO THE NATURE OF THIS SMALL BUSINESS NO REFUNDS WILL BE
OFFERED FOR MISSED CLASSES. SHOULD THE INSTRUCTOR NEED TO CANCEL A CLASS IN THE
EVENT OF AN EMERGENCY, A REFUND FOR THE CLASS WILL BE OFFERED.**

Your Instructor: Anamaria De La Cruz is an honors graduate of Yale University with degrees in
History and Ethnicity, Race and Migration. She holds an MA in Music and is a credentialed CA
educator with over 20 years of experience in the fields of Education and Music, with experience
teaching students of all ages.

Sound Healing: The cost for group Sound Healing Classes is $25 per 45-minute class, or $100 for
4 classes.
